HOW WE HELP
You don't have to figure this out alone.
Whether you're caring for an aging parent, supporting a loved one with a disability, facing a new diagnosis, or simply overwhelmed by decisions — you deserve a trusted partner to walk alongside you.
If you're feeling unknown, uncertain, or overwhelmed...
We'll listen to your story, assess your unique situation, and help identify your care needs, strengths, and opportunities.
If you need guidance and a plan
Together, we'll create a personalized care plan that reflects your goals, values, preferences, and your own vision for well-being.
If life feels out of control...
We'll advocate for your needs, wishes, and rights — ensuring your voice stays at the center of every decision.
If you're a caregiver who needs support...
We'll offer encouragement, practical guidance, and a compassionate presence as you care for someone you love.
If you need emotional or spiritual support...
We'll offer kindness, compassion, and a listening presence that honors the whole person — mind, body, and spirit.

MEET THE TEAM
A professional team in your corner.
Fret Well Care Management is more than one person — it's a team that brings clinical expertise, deep compassion, and real- world experience to every family we serve.
Melissa brings 18 years as a special education teacher and a decade as a hospital and hospice chaplain to her care management work — giving families a true advocate who understands complex systems, difficult transitions, and the humanity in every situation.
Her wife, a Registered Nurse, serves as a clinical resource — giving families access to medical insight alongside care coordination.

The PaJaMaS Framework
At the heart of everything we do is a simple belief: every person — no matter their age or circumstance — deserves to live with Peace, Joy, Mercy, and Safety. These aren't just words. They're the lens through which we approach every care decision.
P
Peace
Reducing the anxiety, confusion, and chaos that often surround care transitions. Creating space to breathe.
J
Joy
Preserving what brings your loved one happiness, connection, and meaning — not just managing their decline.
M
Mercy
Approaching difficult decisions — and difficult people — with grace, patience, and genuine compassion.
S
Safety
Ensuring that every environment, care plan, and decision puts your loved one's physical and emotional safety first.
Nurturing the good, the bad, and the ugly
Let's create a plan that supports your unique well-being. The first conversation is free — no forms, no pressure, just an honest conversation about where you are and where you want to go.
